Patient Family Care Coordinator Evening\Weekend

Primary hours of the position will be a combination of evening/weekend hours with some daytime presence required for meetings, etc.

Position Requirements

1. Two years hospice experience. Management experience preferred.
2. Seven to ten total years experience in related discipline. Current licensure if applicable.
3. Master's degree in one of the core disciplines of hospice preferred.
4. Must have reliable transportation which will enable the individual to perform tasks and responsibilities in a timely and appropriate fashion.
5. Must be able to provide proof of automobile insurance, a copy of which will be placed in the employee's file.
6. Must successfully complete critical thinking skills assessment.
