At Suncoast Hospice, our Patient Family Care Coordinators (PFCC) exhibit team leadership through compassion and caring while exceeding the expectations of the patient/family unit. We are seeking an experienced and compassionate Hospice Team Clinical Manager (Patient Family Care Coordinator) to lead our dedicated team of approximately 25 skilled clinicians and caregivers on our Emerald team, servicing patients in their homes in the Central Pinellas county.

Position Requirements

1. Two years hospice experience. Supervisory experience required; Management experience preferred.
2. Seven to ten total years experience in related discipline. Current licensure if applicable.
3. Master's degree in one of the core disciplines of hospice preferred.
4. Must have reliable transportation which will enable the individual to perform tasks and responsibilities in a timely and appropriate fashion.
5. Must be able to provide proof of automobile insurance, a copy of which will be placed in the employee's file.
6. Must successfully complete critical thinking skills assessment.
