Social Workers are vital to the well-being of our patients and their families as they:
- provide compassionate care to those facing chronic illnesses and come to terms with end-of-life issues and grief
- offer counseling to assist patients and families with decision-making issues and advanced care planning
- work with an interdisciplinary team to assist patients and families reach their goals and move through the stages of grief
- provide case management, linkages to community resources and education to patients and families on a variety of topics
- uphold the commitment of self-determination as the care requests and decisions of patients and families are honored
- serve as advocates for patients and families to help communicate their wishes to achieve their desired outcomes and end-of-life closure
- offer bereavement counseling services up to 13 months after the death of a patient
- respect diversity and build a culture of openness at our Hospice
